Where are you located?
We have two wonderful locations:
Each of our locations maintains its own hours of operation.
You need to carry your health insurance card and picture identity card. If your appointment concerns a medical report, bring it if you have it. New patients should bring all their prescription bottles along with the names, addresses, and phone numbers of previous providers.
Minors need to be accompanied by their parent(s) and/or guardian(s). If the parent(s) and/or guardian(s) is unable to come in with the patient, they may send a signed letter stating that the patient is permitted to be seen in the office alone, along with a copy of the parent’s or guardian’s photo ID.
Yes, we may offer same-day appointments, depending on the provider you would like to see.

Walk-ins are based on the provider’s schedule and their availability. We will make sure to accommodate you the best we can.

If you would like to schedule a doctor’s appointment now, we offer real-time online appointment booking 24 hours a day.

You will not be charged for canceling the appointment, but we would appreciate a 24-hour notice for cancellation. This will help us to keep the office running efficiently for everyone.

There are two options for obtaining copies of your records. Please be advised, you will be charged $0.60 per page for any records over 10 pages.

Call or text 702-616-9471, and tell our Health Information Management that you would like to request your records.

Although walk-ins are welcome, we highly encourage scheduling an appointment in advance as walk-ins are based on the provider’s availability. If you would like to see a particular provider, then an appointment is required. To schedule, give us a call or set an appointment online.

What is primary care?
Primary care is the first point of contact a person has with the health system — the point where people receive care for most of their everyday health needs.
A primary care provider acts as the principal point of consultation for patients within a health care system and coordinates other specialists that the patient may need. As primary care providers, we make sure you get all the services that optimize your health and quality of life, whatever your age or health issues.
Your primary care provider does all the tests necessary to monitor your health and determine when a specialist may be required. Your provider will recommend any necessary medical interventions for your optimal care.

It depends on when your last visit with our office was, and what type of medication is being refilled.

Narcotics will NOT be refilled without an office visit with one of our providers.

Contact our office for any prescription-related questions. Please give our office 48 hours to either complete the prescription or get back to you regarding your request.

What is BHRT, and how can it help me?

BHRT stands for bioidentical hormone replacement therapy. Hormones regulate metabolism and many bodily functions. If your hormones are not at the right level, it can contribute to health problems, including obesity, depression, sexual dysfunction, fatigue, and more.

Supplementation with bioidentical hormones can gently bring you back to optimal levels so you can make progress in reaching your health and well-being goals. Bioidentical hormones are just like the ones your own body makes, so they are safe and effective.

When you sign up for our BHRT program, we perform blood work to test your hormone levels, see where supplementation is needed, and monitor your progress, adjusting the dosages when needed to make sure they are just right for you.

For your convenience, most tests are performed right here at our location, saving you time and keeping your care within a familiar setting. Special tests may be referred to a specialized facility.
Yes, our providers are able to do routine GYN tests, including pap smears, orders for mammograms, STD testing, and more.
Our Henderson location has an in-house LabCorp where our staff can schedule your appointment with them during your visit. For noninsured and self-pay patients, there may be an additional charge from the lab that will process your lab work. For more details, please do not hesitate to call us.
Whether you need to fast depends on what type of blood work will be ordered by your provider. Verify this with one of our medical assistants or your provider.
What forms of payment are accepted?

We accept payment in cash, CareCredit, credit cards, and ATM debit cards with the Visa or Mastercard logo. We also accept American Express and Discover. You also have an opportunity to set up a payment plan; ask our billing department for more detailed information.

We do NOT accept checks for the following: cash, self-pay, and weight loss patients.

For WEIGHT LOSS PATIENTS ONLY, we accept CareCredit as well as the types of payments listed above.

Depending on your insurance, we will either need to collect co-pay, co-insurance, or a deductible. For cash and self-pay patients, we will collect your full office visit payment at the time of service as well as the cost of any procedures done in the office on the same day.
We accept most of the insurances that are PPOs. To see if your insurance is accepted, give us a call or visit this page to view our updated list.
Yes, you can still be seen in our office, but you will be expected to be a cash or self-pay patient. All payments will be collected upfront, including any payments for procedures done in the office. To see a list of our current cash pay prices, please call or text us at 702-616-9471, or visit the FDMC – Self Pay Prices Page.
The Affordable Care Act (ACA), also known as Obamacare, is a healthcare reform law aimed at making health insurance more affordable and accessible. It offers subsidies, called premium tax credits, to reduce costs for households earning between 100% and 400% of the federal poverty level (FPL). The law also expands Medicaid to cover adults with incomes below 138% of the FPL, though not all states have adopted this expansion. Additionally, the ACA supports innovative healthcare delivery methods to lower overall costs while improving the quality of and access to care.
The Marketplace gives you more choice and control over your health coverage. In the Marketplace, you can compare coverage options that fit your budget and needs. In Nevada, our Marketplace is the Silver State Health Insurance Exchange.

Currently, we are contracted with Anthem BCBS and St. Mary’s Health Plans.
Why do I need an MD for my case?
Having a Medical Director on your case will facilitate the approval of necessary studies and the recommendation of physical therapy or chiropractic care. Furthermore, we will be able to monitor the progress and prevent overtreatment or undertreatment. We can also make the initial assessment and better direct your case.
Having an attorney will greatly help in the settlement of your case. If you have not obtained an attorney yet, we would be more than willing to help you find the best attorney for your type of injury.
We see different types of injury cases, anything from motor vehicle accidents, dog bites, slips and falls, food poisoning, and more.
The duration of treatment varies greatly depending on each individual’s unique condition and response to therapy. Some patients may experience significant improvement within a few months, while others may require ongoing care for a longer period. Our treatment plans are tailored to your specific needs and progress, and we will work closely with you to determine the most effective course of action.
How does BHRT fit in with the rest of my weight loss program?

When people have a hard time losing weight despite diet and exercise changes, it could be the result of a hormone imbalance. Hormones affect your metabolism rate and many bodily functions. Bioidentical hormone replacement therapy (BHRT) gently helps bring your hormones back into balance, making it easier to lose weight and helping you to feel better as your body just works better.

Bioidentical hormones are just like the ones your body makes, so they are safe and effective. We monitor the dosage just as we monitor the rest of the program.

We have a physician-supervised medical weight loss program, which includes medications such as HCG, phentermine, and others that can work with your existing medical conditions, all of which will be discussed during your appointment. Your weight loss program will be specifically tailored to your physical condition and weight loss goals.
The average weight loss of our patients is around 2–6 pounds per week. However, every patient is different. The important thing is to lose weight safely and maintain your health through the process.

The HCG program has a strict diet of 500 kcal/day and is only used where medically appropriate.

The diet for the medication program is 1,200 kcal or 1,500 kcal/day.

Our programs are medically monitored so you can maintain and even improve your health through your weight loss.

We also have a nutritionist who can work with you directly if you have any special dietary restrictions.

The medications dispensed will depend on the initial visit. You will be first evaluated by our medical provider before any medication will be prescribed to you. We dispense Phentermine, Tenuate, and Bontril in house at our clinic.
This will depend on how much you want to lose. Our clinic recommends at least three months on the medication program. We encourage and help in weight loss with proper transition to a healthier lifestyle.
The InBody Body Composition Analysis shows the amount of fat versus lean muscle in the human body. The InBody analysis will look beyond the number on the scale and show your body composition proportions: fat, water, muscle mass, and skeletal mass.
Yes, you can exercise while on the medication program. Exercise has many benefits to the body apart from weight control.

Results may be visible after the second session, with maximum results visible 8–12 weeks after the last session.
What Is immunotherapy?
Allergen immunotherapy is a form of treatment aimed at decreasing your sensitivity to substances called allergens. Immunotherapy for allergies involves introducing increasing amounts of an allergen to a patient over several months.
Allergens are selected based on an assessment from your provider and also your reaction to specific seasonal and perennial allergens that you have been tested for.
The course of immunotherapy varies by individual. The initial full course of immunotherapy for allergies is one to three years, but symptom relief can and often does occur after one year of treatment. Completing three to five years of treatment has shown long-term benefits.
Relief from allergies varies by individual and by modality. However, relief can occur within the first few months of treatment and is typically seen within the first year.
Yes, immunotherapy is very effective in greatly decreasing or even eliminating the symptoms of allergies. Up to 85% of allergy sufferers receive a complete cure or a noticeable reduction in allergy symptoms. Read and watch testimonials from many happy patients who have found lasting allergy relief through immunotherapy here.
Yes, most insurance plans cover allergy testing and immunotherapy. We will verify insurance benefits prior to testing.
You will have an initial visit with your physician to discuss the results of your allergy test and why immunotherapy is recommended, at three and six months of immunotherapy, and every six months from then on.
Limiting your immunotherapy to just the spring and summer does not give your immune system time to adapt to allergens that cause you to have symptoms. Immunotherapy injections are required year-round to experience the long-term benefits.
